Notice of Data Breach We have recently experienced a data security incident that may have exposed some of our patients’ personal information. We take the protection and proper use of this information very seriously. We are attempting to reach all patients who might be affected by this personally via first class mail.
What happened? On or about December 12, 2016, we were advised by local police that a storage unit used to house current and former patients’ documents was broken into. The police investigated and determined that seven boxes of patient documents were taken. The documents have since been recovered and inventoried. There is no evidence that the documents were reviewed or used for fraudulent purposes.
What information was involved? The stolen files contained names, addresses, Social Security numbers and limited medical information.
What we are doing. We have thoroughly reviewed the stolen documents. Again, there is no evidence that the documents had been reviewed or used. To help relieve concerns and restore confidence following this incident, we have secured the services of Kroll to provide identity monitoring at no cost to affected patients for one year. Kroll is a global leader in risk mitigation and response, and their team has extensive experience helping people who have sustained an unintentional exposure of confidential data.
